Advance Care Directives

An Advance Care Directive is a document where you express your wishes preferences or instructions for your furture care. It cannot be used for financial decisions. (for financial decisions see Enduring Power of Attorney). An Advance Care Directive replaces Enduring Powers of Guardianship Medical Powers of Attorney and Anticipatory Directions. However if you have one of these documents they remain effective until you create an Advance Care Directive.

When making an Advance Care Directive you appoint one or more Substitute-Decision Makers to make health, residential and personal decisions on your behalf but only in circumstances where you are unable to do so yourself.

Whether an Advance Care Directive will need to be used will depend on your personal circumstances which often cannot be predicted. Accordingly, creation of an Advance Care Directive provides peace of mind for the future.
